Worksheets and Other Forms to Download
Hello! We've developed several worksheets for our students, as well as
assignment sheets, grade pages, report cards, and various other forms. We'd like to share these with you. It may take me a
while to get them all transfered into pdf format, but I will post them as I get them done. To use, simply hit print, or
if you plan to use it multiple times you can save it to your hard drive. I do ask that you do not post it on your own web
site without first obtaining my consent, and please do not claim any of these forms or worksheets as your own.
| Weekly
assignment sheets can be a great way to help your students keep track of what they need to do. You can also print up
several, punch holes in them, and place in a binder to use as a lesson planner. Our weekly assingment sheet has spaces for
13 subjects, as well as spaces for all five school days, a check off box to let the student know when they have finished
that subject for the week, and a box for the teacher to record behavioral problems.

| Our grade pages allow you to keep track of your students'
progress. Each page has spaces for 20 days and 13 subjects. Print on front and back of pages and punch holes to create your
own grade book. Each page is intended for a single student.

| If you have multiple students or just prefer to keep to a
rigid time table, this is the perfect way to help your students know what's to come. 16 time slots for each day Monday -

| Our tooth and exercise charts are a great touch for grade
school level health and gym classes. The tooth chart is a basic chart with spaces for the date as well as spaces to mark
off each time (up to 3 times a day) your students/child brushes their teeth. We used it as part of our first grade health
class. Designed for a classroom environment with spaces for student name and parent's signature.

The exercise chart is divided into three basic charts with 8 columns and 10 rows each. All spaces are blank so it can be
used in a number of ways. Use in a classroom environment to record the weekly physical activity of 3 different students,
use a seperate page for each activity and record the daily duration for up to 9 students, etc., etc.

| Our gym report is the perfect way to keep track of middle
school students gym progress. Divided into 5 daily sections - each section has a space for the date, uniform,
participation, up to 5 seperate activities, and a daily total. Print on front and back of pages and punch holes to create
your own gym grade book. Each page is intended for a single student.

| A great way to teach students responsibility as well as
promoting good hygine is to grade them on their appearence. This works especially well with middle school students. Our
uniform and hygine report is set up to record a single student's appearence for one week. Seperate spaces for date, if the
uniform (when applicable) was worn, cleaness, undergarments (especially good for middle school girls), hair, overall hygine
(deoderant, nails, etc.), and teeth, as well as a space for the daily total. Print on front and back of pages and punch
holes to create your own hygine grade book. Each page is intended for a single student.

| We hope to have more foreign language worksheets up soon,
but here's what we have for now. Our foreign language worksheets are directed towards elementary grade levels.

| Much to the dismay of any teacher, there are students who
would prefer to cheat than to study for a spelling test. As a way to prevent this, I have developed a mid-week spelling
quiz and end of week spelling final worksheet. Both have spaces for unit, date, and student name, as well as numbered
spaces for up to 20 spelling words. Print right before the test and hand out.

| Just in time for Mother's Day! I'm sure you have seen them.
When I was growing up, every Kindergarten class made these for Mother's Day, as well as several Sunday School classes
(though the wording was a little different). When I decided to homeschool my daughter, I felt I was losing out on this
little tradition so I designed my own blanks. Here for you are 4 different styles of the "Fingerprints" poem - all set for
Kindergarteners with a space for their hand print. If you'd like the wording to be that of the original poem and suitable
for any age, just e-mail me. I'm not sure who originally wrote the poem, but it is one I find to be very touching.
